Superior app to boat trader

I used the boat trader app exclusively but once I used this app, I haven’t gone back to boat trader. This app is not glitchy and I love the way you can constantly scroll down even after swiping through pictures of listings. I would like a mileage radius feature though - unless I’m missing something I can only choose my location, (unknown distance???) or I must choose a single state. Choosing a single state seems silly to me as I’m in the northeast and want to see several states inventory at a time. If these selections exists then I’m missing them or it’s not intuitive- but over all, I do love the app.

Technical failure

I wouldn’t be so harsh if YachtWorld didn’t have a de facto monopoly on the used sailboat market. This app has one simple job: show a list of products, show details and pictures of individual products when clicked on, and most importantly - keep track of where the user was in the list when going back to the list. Instead we get pushed back to the top of the list every time because the developers can’t solve this monumental conundrum of computing? My peeps just store and update one variable representing the highest seen boat while scrolling and keep track of it, then show me that place in the list whenever I go back. If that’s too much, then when I’m viewing a specific boat and hit back to go to the list, at least put me at the place in the list where that specific boat is instead of the top. Also the filters don’t match the website (website is by region whereas app is by state). Pictures often don’t display correctly. Often I get “no results” for a search even though the blue button after setting filters says there will be results. Change one filter, search, change it back, search, and suddenly I now have results. Just pure untested jank.

Decent source

I wish you could filter out certain things. Such as time share boats / fractional ownership or other items not interested in. Also some listings are really vague or have limited information about the boat they wish to sell. This is a loss to those trying to sell and not a fault to the developer. But what if the the developer could add fields to help encourage customers to add more data about there boats at the time they write up their ads (something to get better info). It seems weird that someone who wants to sell an expensive yacht but doesn’t make the effort to add as much detail as possible. Update: Dropping to 1 star. Perhaps not the fault of the app but I emailed Lelievre / Alize Marine from within the app. Shortly after i started getting really bad spam. I never ever had spam mail on that email before. I also never got a reply from that marine company either. Not sure why they would post an add with pictures that were clearly not the pictures of that boat or older pics. The description said the boat was hurricane damaged but the pictures were of a new boat. If i am wrong let me know and i will either update or delete my review.

Melissa Red Hoffman was “feeling really stuck” last summer. A 50-year-old surgeon in Asheville, N.C., Hoffman had been struggling with long COVID since getting infected with the coronavirus two and a half years earlier. “Deafening fatigue” was one of her worst symptoms, she says. “I feel tired behind my eyes from the moment I get up to the moment I go to sleep.” She managed to work part time, but much of her work had shifted to administrative tasks that she did from her couch.

“I was really at a point where I had tried so many different things myself, with so many different providers,” she says, “not really sure what the hell to do next.”

Then she found Remission Biome . It’s a research project started in early 2023 by Tamara Romanuk and Tess Falor, two people with myalgic encephalomyelitis/chronic fatigue syndrome, or ME/CFS, a chronic disease that shares symptoms with long COVID. Project participants have taken medical research into their own hands to determine whether and how changes to their gut bacteria can improve their health. After an initial test with three participants led to some symptom relief, Romanuk and Falor announced last July that they would recruit 50 people with ME/CFS, long COVID or both for a larger test of the project’s protocol.

Hoffman was one of 500 people who applied within 36 hours of the call for volunteers. By the fall, she and 49 other people, dubbed the “Renegade 50,” had joined the project.

Remission Biome’s protocol is a multistep process, which participants undertake in consultation with their physicians. Initial steps involve patients collecting samples of their guts, immune systems and other connected organ systems, either at home or at a health care provider’s office. After those samples are analyzed by a lab to get baseline data, participants take a regimen of over-the-counter supplements, such as probiotics to cultivate certain types of gut bacteria, and then a prescribed antibiotic. Next comes further testing to examine if and how the regimen altered the composition of the gut microbiome. Throughout the process, participants track their symptoms and learn about past research on the microbiome that informed the project, ensuring that they understand the rationale for every step.

Early in the testing process, Hoffman’s fatigue started to lift, she says. “That’s been exciting, just to feel a little bit of a change.”

Alleviating symptoms — which can include debilitating fatigue, trouble sleeping, intense allergic reactions and cognitive problems — motivates many members of the Renegade 50, who come from different countries, age groups and stages of illness. But participants also aim to collect and publish data that will give the broader scientific community more information about ME/CFS and long COVID, two complex, often fluctuating conditions.

Participant María Richardson, a 36-year-old former educator in Mexico City, has dealt with progressively worse ME/CFS symptoms since high school. She received her diagnosis in the United States in 2015, but when she moved back to her native Mexico, where knowledge of the condition is limited, trying to get care “was like starting from zero,” she says. Remission Biome helped her better understand her own symptoms and share scientific information with the ME/CFS community in Mexico, through the ME/CFS advocacy group Millions Missing Mexico .

Remission Biome is one effort in the growing movement of patient-led research, which seeks to investigate chronic conditions that have been under-researched by academic and clinical scientists yet impact many people’s lives.

“People who were ignored by the American health care system … often need to turn to each other in order to gather the data that gets the attention of the mainstream,” says health care researcher Susannah Fox, author of the new book Rebel Health: A Field Guide to the Patient-Led Revolution in Medical Care .

Compared with mainstream medical research that tends to focus on finding biological causes and disease cures, patient-led work is more often rooted in what’s immediately relevant to patients’ daily lives, like identifying symptom triggers or relievers. But the approach faces challenges — particularly a lack of funding and other research resources — as scientific institutions aren’t set up to support these projects.

Patient-researchers and their scientist collaborators say the patient-led approach has big potential to move chronic disease research forward, making it more informed, quicker and more poised to directly improve patients’ lives.

Projects like Remission Biome “are going to change how research into these chronic, multi-organ-system diseases is going to be done,” Hoffman says. The approach may someday become a standard part of more mainstream research.

Experimental design

Remission Biome is studying whether changes to the gut microbiome can improve the health of people with ME/CFS or long COVID. Participants follow a protocol that includes taking prebiotic and probiotic supplements and a course of antibiotics.

Initial safety checks and testing : Samples from several body systems are collected and analyzed to ensure it’s safe for a volunteer to participate and to collect baseline data.

Symptom tracking : Participants record their symptoms throughout the project.

Start supplements : Participants begin taking prebiotics and probiotics.

Antibiotic added : An antibiotic is added to the regimen for 14 days.

Post-antibiotic testing : Body systems are checked again to look for changes.

Result : Remission Biome members hope to experience some symptom relief, as well as publish study findings and inspire new research projects for professional scientists.

A history of patient activism and patient-led research

About 1.3 percent of adults in the United States have ME/CFS, according to the U.S. Centers for Disease Control and Prevention. Scientists first noticed the condition in the 1930s, but since then, it’s been hard to define and hasn’t attracted extensive research attention. Initial observations noted outbreaks characterized by fatigue, chronic pain and other symptoms now associated with ME/CFS, often occurring — but not always — after viral infections. Scientists started to link these mysterious outbreaks in the 1980s under the umbrella term chronic fatigue syndrome.

Progress on identifying the disease’s triggers has been slow, in part because of the wide variety of symptoms across many organ systems and in part due to relatively limited research funding. And some doctors have dismissed patients’ symptoms as all psychological — a factor that some experts connect to the disease’s higher burden on women.

Combined, these challenges have contributed to a lack of treatments for people with ME/CFS, despite the illness’s potentially devastating impact on patients. Long COVID — which 6.8 percent of U.S. adults currently have, according to data from the CDC and U.S. Census Bureau — raised the profile of ME/CFS during the pandemic because of the two conditions’ similarities ( SN: 3/4/24 ).

“Biomedical research has blind spots.” Susannah Fox

Remission Biome started thanks to a Twitter conversation in fall 2022. Falor and Romanuk realized they had both independently experienced what they call “remission events,” in which symptoms recede for a few hours or days after courses of antibiotics. These events led each of them to look into the possible connection between ME/CFS symptoms and the gut microbiome, an emerging area of study with many unanswered questions. The pair were also both working scientists before their symptoms became debilitating. Falor had worked as an aerospace engineer at NASA; Romanuk had been a biologist studying microbiomes.

The two scientists set out to replicate their remission events — and collect extensive data on how their microbiomes and bodily systems changed to better understand the underlying biology of these events. They started with a self-test in early 2023, which included taking a lengthy list of supplements chosen to either increase or decrease levels of specific bacteria with possible ME/CFS connections. In addition to Romanuk and Falor, Isabel Ramirez-Burnett, a 50-year-old engineer and health coach in Rhode Island who has lived with ME/CFS since childhood, participated in the experiment.

The testing “went even better than we could have expected,” Falor says, with two of the three participants experiencing remission events. So Remission Biome expanded to the Renegade 50 cohort and fundraised through a crowdfunding campaign, grants and sponsorships to support this larger project. The team also recruited the participants’ physicians, to help ensure safety, along with scientists to collaborate with the participants and other volunteer researchers working on the project. Scientists regularly attend research meetings hosted by Remission Biome, Falor says, which include presentations and discussions about new, relevant findings in other ME/CFS and long COVID research.

Theoharis Theoharides is one of those scientists. As director of the Center of Excellence for Neuroinflammation Research at Nova Southeastern University in Clearwater, Fla., he has decades of experience studying mast cell activation syndrome, a chronic condition characterized by intense allergic reactions that is often diagnosed alongside ME/CFS and long COVID. “They’re very bright, very dedicated,” Theoharides says of Falor and Romanuk. He has provided feedback on Remission Biome’s regimen of supplements and plans to help analyze microbiome and blood samples taken from the Renegade 50 participants to look at how immune system changes may connect to their gut bacteria.

Another collaborator is Tatyana Dobreva, cofounder and CEO of the San Francisco–based biotech start-up ImYoo , which operates remote clinical trials and other research. ImYoo is assisting Remission Biome with genetic analysis of patient blood samples. The Renegade 50 study is similar to other ImYoo projects studying conditions such as IBS and sickle cell disease, in which participants tie symptom tracking to data from medical testing, Dobreva says.

Remission Biome adds to a long history of patients with complex and contested illnesses advocating for their communities, Fox says. “Every decade of the 20th century had an example of people who were either being ignored or who were being discriminated against” by scientists and doctors, and who “banded together to innovate or gather data,” she says. Examples include Black people with sickle cell disease in the 1970s and people with HIV/AIDS in the 1980s ( SN: 12/8/23 ). In some cases, this translated to patient-informed research, in which patients consult on scientific projects, informing everything from research questions to how results are disseminated.

In the 21st century, the internet aided patient-led projects, with patients actually doing research, as like-minded patients could more easily find each other, as happened with Romanuk and Falor, Fox says. In these projects, patients also closely follow scientific studies about their disease and may collaborate with academic experts to develop scientific frameworks, rather than self-experimenting individually.

ME/CFS patients have been particularly motivated to pursue their own research, says Emily Taylor, vice president of advocacy and engagement at the ME/CFS organization Solve M.E . One key motivator is “the failure of the medical establishment to provide any sort of support or treatment or quality of life improvements for this population,” she says. Previous ME/CFS research done without patient input, such as a now-debunked clinical trial examining exercise as a potential treatment, has led patients to push back with their own studies.

“There was a desperate need to validate the anecdotal stories of patients in a formalized way,” Taylor says.

In spring 2020, during the first months of the pandemic, patients whose symptoms persisted for weeks after the initial infection started documenting their complex symptoms in real time. The Patient-Led Research Collaborative , or PLRC, formed out of a long COVID support group, led by members who had scientific experience.

PLRC released its first report in May 2020 , documenting symptoms common among the group’s hundreds of members. “We saw a need to start collecting people’s experiences and really try to take things into our own hands,” says PLRC cofounder and long COVID patient Lisa McCorkell.

Patient-led and patient-informed research can be a win-win for both patients and scientists, advocates say. For patients, this work is more likely to address questions that are meaningful to their daily lives, says Jaime Seltzer, director of science and medical outreach at the advocacy group #MEAction . In one pre-pandemic example, a patient group focused on polycystic kidney disease proposed potential treatments to scientists at the University of Cambridge, leading to clinical trials at a new patient-led research hub.

Patient leadership can also inspire people to participate in clinical trials, as the interest in joining Remission Biome demonstrates. And study designs informed by patient experience often prioritize accommodations for people with different levels of symptoms or access to care, meaning a more diverse group of patients may be able to participate. With a patient-led, “decentralized” approach to research, “we can reach more people in more diverse areas” who don’t live near medical facilities in big cities or aren’t able to travel for clinical trials, Dobreva says.

Connor, a member of the Renegade 50 who asked that only his first name be used to maintain medical privacy, “couldn’t participate in a traditional study,” says his wife, Nicole Bruno. Since a COVID-19 infection two and a half years ago, he has faced a severe case of both long COVID and ME/CFS, leaving him bedbound in a dark room.

“He could never go to a lab” or a doctor’s office to have samples collected, Bruno says. But with Remission Biome’s remote framework and individual support, he can be a patient-researcher. In addition to flexibility in locations, each member of the cohort is going through the testing protocol at their own pace, incorporating their microbiome test results, other diagnoses and input from their physicians. Flexibility also helps with logistical challenges; for example, test kits take longer to ship internationally.

For scientists, patient-led studies may move a field forward by highlighting key questions and hypotheses that might not emerge from traditional research. “Biomedical research has blind spots,” Fox says. McCorkell points to a paper from the PLRC , published in eClinicalMedicine in 2021, that expanded upon its 2020 survey work by describing 200 long COVID symptoms across 10 organ systems based on a detailed survey of about 3,800 people.

“It is still, to this day, one of the most cited papers in long COVID,” McCorkell says. Without this paper, she adds, other scientists might still be investigating “a small, limited set of symptoms” rather than the full scope of the condition. David Putrino, a long COVID clinician and director of rehabilitation innovation at Mount Sinai Health System in New York City, also points to the PLRC paper as an example of successful patient-led research that informed later studies.

Patient-led research “moves orders of magnitude faster than traditional modes of research,” Putrino says, because it focuses on the questions that are of greatest concern to patients, leading more quickly to impactful results. Patient-led groups may also be able to start new studies more quickly than institutions that have to, say, go through formal academic procedures, he says. In that way, this research is similar to how start-ups move faster than large corporations.

In addition, patients can help scientists design studies that are more likely to provide accurate results. For example, feedback from members of Remission Biome and other patient representatives helped David Esteban, a biologist at Vassar College in Poughkeepsie, N.Y., who was looking for people who had gotten COVID-19 but didn’t develop long COVID and could serve as control patients in a project funded by PLRC.

“Their perspective was, many people who recover from acute COVID go through a period where they feel better, but then get worse again,” he says. “I hadn’t really thought about that.” But that insight helped Esteban establish how long after a COVID-19 infection to wait before declaring a patient past the threshold for developing long COVID.

After studies are completed, patient teams may be more thoughtful about communicating results back to patient communities. In sharing a recent paper about managing ME/CFS that she co­authored with clinicians at the Mayo Clinic in Rochester, Minn., for instance, Seltzer anticipated questions that ME/CFS patients might have about the study. She explained up front that the paper was a concise review and could not include every relevant study, as patients would want to know “why I hadn’t mentioned their favorite paper,” she says. Such communication can “save a research group a lot of time and energy,” Seltzer says, and can encourage patients to bring the paper to their doctors so that the findings might inform their health care.

Groups like PLRC are working to build infrastructure to help scientists better engage with patients, including experienced patient-researchers and others who haven’t done scientific work before.

In January 2023, PLRC and the Council of Medical Specialty Societies introduced scorecards for academic teams interested in these collaborations. The scorecards can help teams evaluate success. “Our scorecards were developed with the intention of trying to change the baseline of what’s considered acceptable patient engagement,” McCorkell says, moving away from “tokenizing” engagement that she and other PLRC members have experienced. Taylor, at Solve M.E., would like to see the scorecards or a similar evaluation incorporated into traditional funding applications at scientific institutions.

The challenges of patient-led research

Current institutional and financial support for patient-led research projects is limited. These projects typically are not eligible to apply for academic and government grants, leading them to seek money from nontraditional sources. Patient-researchers also don’t tend to have access to laboratory space, clinical tests and other research resources.

“We’re limited in the type of research that we can do,” McCorkell says. As a result, surveys and self-experimentation are the most common methods.

Internal capacity is another challenge: Chronically ill people tend to have limited energy to devote to projects; they must balance this work with managing their symptoms. Patients tend to be more ambitious than their available energy can support, Seltzer says. Sometimes a patient-researcher might have to take a break from a project to recover from a symptom flare-up. Projects like Remission Biome take these crashes into account when designing experiments and distributing tasks.

“If I disappear for a week,” it’s OK, says Katrin Boniface, a doctoral student studying the history of horses at the University of California, Riverside who had her own remission experience before joining the Renegade 50. But these constraints might frustrate academic or clinician collaborators who want patient-researchers to answer emails at all hours or pull together a last-minute grant proposal.

Nonpatient scientists might also be skeptical of results from patient-led research, as many in the scientific community haven’t yet recognized how lived experience can improve studies, Seltzer says. Although many patient-researchers have scientific backgrounds, they might not be experienced in biomedical research, leading to perceptions that they are underqualified and that their work is not rigorous or may even be biased.

Advocates like Seltzer argue that patient-researchers are more incentivized than anyone to make sure their results are accurate. “If we’re wrong, we and people like us suffer,” she says.

Taylor argues that data from patient-led research should be added to the types of evidence that regulatory agencies like the U.S. Food and Drug Administration consider for approving treatments. The FDA and the National Institutes of Health took one step in this direction earlier this year by soliciting data from long COVID patients and doctors about their experiences with treatments approved for other diseases.

But some scientists and doctors are concerned that patient-led projects might encourage some patients to self-experiment on their own without appropriate safety measures. This has been a big challenge for Remission Biome, especially after its members posted about remission events during the project’s first phase in early 2023. Initially, the plan was to openly share all aspects of the project, including protocols and results, says Ramirez-Burnett, one of the three early participants. “But then we realized that people were starting to pick pieces of the protocol in order to do it, which is not safe,” she says. “So we had to close that document.”

Now, when asked about the full protocol, as they often are on social media, Remission Biome participants typically encourage safety and emphasize that more testing is needed before it’s widely shared. In the future, Ramirez-Burnett hopes to educate more clinicians about the project so they can work with patients outside the Remission Biome infrastructure.

Patient-led projects may also struggle with logistics. This has been the case for Remission Biome. Its two founders split in December over disagreements about the project’s pace, its handling of safety aspects and how to incorporate the project as a formal business. As a result, Romanuk and the group parted ways.

The Renegade 50 test was put on hold until mid-March while Falor and other project members addressed this leadership change and set up as a nonprofit, she says. The team is also adding more safety steps and participant education on the antibiotic in the testing protocol because that antibiotic may have negative side effects for some people with ME/CFS. Falor expects the Renegade 50 phase will be completed later this year, after which the project will share preliminary results and begin setting up a cohort of 500 participants.

Tests and supplements for that next cohort will require more financial support, which Remission Biome will continue to raise from its GoFundMe campaign and grants. The project has also secured sponsorships from supplement and testing companies, such as the probiotics provider FitBiomics, to provide research supplies to participants. Financial support is especially important for participants living in places where it’s difficult to receive medical care for ME/CFS, says Richardson, the Renegade 50 member in Mexico. Many patients globally could benefit from this work, she says.

Remission Biome is also working toward scientific publications, based on data from the Renegade 50 cohort and from side projects. But the 50-person test might not lead to publishable results, says scientist-collaborator Theoharides. The microbiome is extremely complex, and, unlike a clinical trial, the Renegade 50 group does not include control patients not taking the treatments. But he hopes “the information that will come out of this study might actually give us some new directions.” One key advantage, he says, is that each participant is testing many supplements rather than focusing on one at a time; ME/CFS and long COVID are such complex diseases that it’s unlikely for a single treatment to work for all patients or have a lasting impact.

Esteban, the biologist at Vassar College, similarly hopes to examine how different antibiotics might work together to alleviate symptoms. “I’m already thinking about experiments that I could do,” he says, such as work in lab animals that would “start to explore some of the proposed mechanisms that might underlie the effects they’re seeing with the antibiotic treatments.”

While Remission Biome’s participants are excited to contribute to research, their most important goal is to provide “solutions for the ME/CFS community,” Ramirez-Burnett says. “So people don’t have to lose their jobs, lose their relationships, not get proper care.”

Among the three Renegade 50 participants who had completed the testing protocol as of January, one experienced a remission event, signifying a potential success, Falor says. Meanwhile, the project’s frequent meetings, Slack group, apps for shared symptom-tracking and other communication options could provide models for other patient-research efforts.

Remission Biome participants who have dealt with ME/CFS for a long time, like Richardson, feel particularly motivated to help find answers for the millions around the world newly struggling with long COVID. “People with mild long COVID sound like what I experienced 20 years ago,” Richardson says. She hopes that the lessons learned from Remission Biome and other projects like it can help prevent new long COVID patients from experiencing decades of symptoms.

The Medical Research Archives is the official journal of the European Society of Medicine. The journal was founded in 2014 and has published over 2,500 peer-reviewed articles across 11 volumes. Originally published both in print and online, the journal has since become a fully online and open-access publication. Issues are published monthly on the society’s website and on a variety of digital platforms.

The journal publishes original research, reviews, and case reports addressing health issues of interest to a global community of medical professionals. While issues of the journal are often focused around topics of particular interest to the society, submissions from non- members are most welcome. In addition to the regular issues there are also several theme issues released each year which are dedicated to a single disease or topic.

Public health has always been a core focus of both the society and the Medical Research Archives. The society’s scientific resources translate into compelling advocacy messages to inspire change in healthcare policy and medical innovation. Reducing the burden of disease is a difficult challenge which often demands a multidisciplinary approach. Our History

The Stowers Institute for Medical Research was born from the vision of our founders, James E. Stowers, Jr., and his wife Virginia, who aspired to provide an environment of extraordinary scientific research.

Jim trained as a physician before launching and steadfastly nurturing American Century Investments into a highly successful, global asset management firm. Virginia had a career as a registered nurse. So, the couple had a profound understanding of the role that foundational biomedical research plays in generating new therapies and saving lives.

But it was their personal experiences with cancer that solidified their decision in 1994 to dedicate their personal fortune toward creating one of the world’s most influential centers for foundational research.

Over the next decade, our founders endowed the Institute with gifts totaling over $2 billion, and established a funding model whereby the science is funded through the dividends from American Century Investments, placing these visionary philanthropists in the top ranks of the world’s most generous givers.

Medical information (MI) professionals are primarily responsible for researching and responding to unsolicited requests for information on their company’s product(s). In an effort to set a standard for quality, the Pharma Collaboration for Transparent Medical Information (phactMI) created a code of practice for the provision of medical information to healthcare professionals. This code introduced the term “MI science skills” to describe the expertise required to perform the duties of an MI professional. These skills can be summarized by the acronym DRESS. In order to effectively and efficiently respond to an unsolicited request for information, the MI professional essentially follows five steps: define the question, research the topic, evaluate the evidence, synthesize a response, and share the answer. As this approach mirrors the scientific process for data generation, MI scientist may be a more apt description for this role. This paper explains the rationale behind the term MI scientist and the skills associated with each component of the DRESS approach.

Introduction

Effectively responding to a medical information inquiry is akin to the scientific process [ 1 ]. The Pharma Collaboration for Transparent Medical Information™ (phactMI) was formed in December of 2014 to support the safe and effective use of medicines. One of the first acts of phactMI was to develop the “medical information code of practice (COP) for responding to healthcare professionals (HCPs) requests.” Published in 2015, the COP defined three fundamental elements for medical information (MI) practice: clinical and pharmaceutical expertise, scientific balance of medical responses, and quality standards. The COP also introduced the term MI science skills to describe the technical expertise required of MI practitioners. As stated in the code: “MI professionals should be trained in MI science skills, including literature searching, literature evaluation (i.e., study designs, statistical methodology, and clinical product/device training), information synthesis, and medical writing. MI professionals should be able to critically evaluate scientific literature, formulate a medical response, and effectively communicate the information to the requesting HCP to assist them in making a clinical decision [ 2 ].”

The term MI science Skills was selected because the process by which an MI professional fulfills their duties for responding to unsolicited medical information inquiries is generally analogous to the scientific process. Stated simply, the scientific method starts with a question, followed by experimentation, analysis/interpretation, and publication or communication of results [ 3 ]. Similarly, answering an MI inquiry starts with understanding the question, followed by obtaining relevant evidence to answer that question, then critically appraising or evaluating the evidence, and finally communicating an answer in writing or verbally. Each of the steps in the MI process requires certain expertise and hence the term MI science skills was born. By the same rationale, those practicing MI science skills can therefore be referred to as MI scientists. In this paper, we describe in more detail the aspects of these MI science skills exercised by MI scientists.

Case Studies

The primary responsibility of an MI scientist is to research and respond to unsolicited requests for information on their company’s product(s). The inquiries MI scientists receive and respond to vary in terms of nature and complexity. Some questions are straightforward and tend to be answered directly from a product’s prescribing information. For example, Dr. Jones is a cardiologist requesting information regarding the recommended titration schedule for a patient starting product X, a hypertension management medication. Requests that are less complex in nature may also include topics, such as recommended dosing, mechanism of action, medication properties, and approved indications. Complex inquiries require more effort as the MI scientist must utilize their MI science Skills to research and develop a tailored response to the question at hand. Requests that are complex in nature can include topics, such as off-label use, alternative dosing and administration, use in special populations, use with other medications, and others. For example, Dr. Doe is a hospital pharmacist that works closely with his institution’s surgical team. He is requesting information regarding the use of product B, an intravenous corticosteroid, as an alternative for opioid analgesics in a 55-year-old male patient undergoing spinal surgery. As this information is not readily available, the MI scientist must utilize the skillset described in this paper by the acronym DRESS (define the question, research the topic, evaluate the evidence, synthesize a response, and share the answer) to research and respond to this inquiry.

Define the Research Question

The role of an MI scientist requires responding to specific questions with truthful, non-biased, scientifically accurate, and current medical information. Their clinical expertise on the product(s) and specific therapeutic areas is required to help better understand and define the query. The MI role requires great attention to detail, so the response can be tailored to the specific question asked. Watanabe et al. presented a 5-step systematic approach for how to respond to drug information requests [ 4 ]. The five steps were to classify the request (for example: is this a safety question or a dosing question, or both?), obtain background information on the request, conduct a systematic search, develop a response, and reclassification (to determine if the response aligns with the initial classification of the request). Kirkwood modified the approach to 7 steps that involves the following: identify requestor demographics; obtain background information of request; determine and categorize the specific question; develop and implement a search strategy; evaluate, analyze, and synthesize the data; formulate and provide a response; and follow-up and document [ 5 ]. Using a systematic method supports answering a complex and difficult question with an appropriate response. Straightforward questions also need further clarification, as there may be more background information to help better answer the ultimate question.

It is essential to know the requestor’s demographics. This may provide insight into the requestor's level of sophistication and knowledge regarding the subject matter. For example, is it a patient or a pharmacist or a physician? Understanding the context of the question can help to better formulate an appropriate response [ 4 , 5 ]. The MI scientist asks the appropriate clarifying questions that can lead to a deeper understanding of the true nature of the requestor’s inquiry and confirms the research question. The MI scientist realizes that the query may not have been asked very concisely or clearly. They also recognize that the responses to the query may differ depending on the patient’s medical history or other special circumstances. There are two key aspects to understanding the question: (1) It is important to find out if the question is specific to a patient’s condition or if it is truly a question for the requestor’s own knowledge. (2) It is also good to know what resources or what search strategies the requestor already used. This can avoid duplication of effort or an opportunity to clarify misinformation and validate appropriate sources of information [ 4 – 6 ].

Suppose the requestor asks a general question on the dosing for a drug. In this case, it is important for the MI scientist to further clarify the context, especially for a drug that may have multiple indications. It is best practice to confirm with the requestor the ultimate question before the MI scientist creates the search strategy [ 4 , 5 ].

Research the Topic

The ability to conduct an effective literature search is one of the critical steps in achieving competency in evidence-based medicine and a key competency for an MI scientist [ 7 ]. Once a well-defined research question has been created, the next step is to create an effective search strategy by which to find the data to answer the question. The strength of a scientific response depends on the data used to support it. Literature stressing the importance of searching skills can be found in publications on evidence-based practice in medicine [ 7 – 9 ]. Although there are a limited number of studies, there is evidence that many healthcare professionals feel they could be more proficient in literature searching and that formal training may improve overall search and retrieval skills [ 9 , 10 ].

A search strategy encompasses several steps and considers the requestor, the breadth of the topic, and the availability of databases. Based on the research question, the MI scientist will identify the search terms to use and how these terms will be combined. For example, will the search use a Boolean approach (OR, AND, or NOT)? Alternatively, will free text be used or will a closed vocabulary such as medical subject heading (MeSH) terms be applied [ 11 , 12 ]? Various searching tools can assist in the development of appropriate search strategies. For example, the PICO tool emphasizes searching by the population, intervention, comparison, and/or outcomes, while the modified PICOS tool adds in study design [ 13 ]. Based on the MI scientist’s skills and knowledge, the search strategy will be developed and executed. Most commonly used databases provide a search-builder function to assist the researcher [ 11 , 12 ].

There are numerous databases of medical literature, each designed with a specific purpose. See Table ​ Table1 1 for examples of some commonly used literature retrieval databases [ 14 – 19 ]. An MI scientist may also refer to guidances set forth by regulatory bodies, such as the US Food and Drug Administration (FDA) and the European Medicines Agency (EMA) as reputable sources of information. By becoming familiar with available sources, the MI scientist can develop a more effective search strategy. In the current era, we are fortunate to have access to many free and subscription-based high-quality electronic databases. A particular institution or company may subscribe to one or more of these databases and it is important to be acquainted with each one to understand what they offer [ 20 ]. It is also important to only utilize reputable databases to avoid misinformation such as evidence that may exist in predatory journal articles which have not undergone the same peer-review scrutiny [ 21 ].

Examples of commonly used literature retrieval databases [ 14 – 19 ]

MI scientists may take advanced biomedical literature search training to enhance their skillset; this may include connecting with a skilled librarian who can provide instruction on working with various databases, using indexing, and developing advanced search strategies. For non-governmental databases to which an organization subscribes, it is often possible to contact the owner of the database for training. Once the MI scientist has conducted a thorough literature search, the next step in the process is to evaluate the literature.

Evaluate the Evidence

Critical appraisal/literature evaluation skills are key competencies for an MI scientist. Depending on the nature of the medical inquiry, there may be a wide variety of literature available. It is important to remember that not all study designs are created equal. Hierarchies of literature can be found in a variety of references and resources. The Oxford Centre for Evidence-Based Medicine provides a thorough background and explanation of their version of the levels of evidence [ 22 – 24 ]. Based on the question that is being answered, levels of evidence can differ. It is essential to utilize the highest level of data available to answer a question. For example, if there are plenty of available randomized controlled trials, it is unnecessary to summarize lower quality trials or case reports/series. However, if there is only observational data available, then the response document would be reflective of that [ 25 ]. The MI scientist must be familiar with the levels of evidence and types of study designs. Table ​ Table2 2 provides a high-level summary for easy reference.

Based on the quality of any of the studies, the level of evidence can be shifted down. It can also be shifted up if there is a large/very large effect size

Additionally, most MI departments have their own guidance documents and standard operating procedures (SOPs) around the development of a written scientific response. These guidance documents provide a framework of elements to include in each section of a scientific response document. Based on the inquiry, not all elements are necessary for all documents. Realistically, it is not possible or always appropriate to include all publications from a literature search. In fact, an MI scientist needs to use their professional expertise and judgment to develop concise, accurate, scientifically balanced, non-promotional, and evidence-based scientific response documents.

Once all relevant references are retrieved, a critical evaluation is next. However, most busy practicing HCPs, including those in the pharmaceutical industry, do not have the luxury of time to critique each article in a “journal club” fashion. While it is important for MI scientists to have a working knowledge of appropriate reporting items for various trial designs, practically speaking, it is helpful to have a concise and quick method to critically evaluate the literature. Table ​ Table3 3 summarizes the brief questions that help in evaluating a clinical trial [ 26 – 28 ].

Questions to ask when summarizing a clinical trial [ 26 – 28 ]

The statistical analysis section of the article is usually overlooked due to a poor understanding of statistics [ 29 ]. While most individuals remember that the p value needs to be below 0.05 to be considered statistically significant, few realize this is an arbitrary number and there is a push to move away from a p value of 0.05 [ 30 , 31 ]. In evaluating a clinical trial results, it is important to not only look at statistical significance but also the clinical relevance of the results. Is there a ‘so what?’ For example, based on the question posed in the case studies section of this article, if intravenous corticosteroids could statistically significantly reduce the use of opioids post-surgery, you would likely recommend them. However, if the reduction was only 0.5 mg morphine over a 72-h period, while this may be statistically significant, it may not be clinically relevant. So, the response should mention the decrease, but highlight the limitations.

Synthesize the Response and Share the Answer

Professional medical writing and verbal communication skills are important for the delivery of a MI response to help the HCP make an informed clinical decision. This includes restating the relevant background information and providing a summary of the literature, search strategy, and references used [ 32 ].

Recently, phactMI published a proposed best practice guideline that details the sections of a scientific response document [ 25 ]. The MI scientist can use these best practices to present the written response documents in a consistent format that is user-friendly for the intended audience. The methods and types of communications are tailored to the individual based on factors such health literacy and depth of knowledge about the question at hand. Each response is curated to meet the unique needs of the requestor, whether he or she is a patient, caregiver, or HCP and with consideration for their level of specialization and knowledge. An MI scientist understands that they do not provide recommendations or advice to HCPs or patients. Their responses should be written in a way that does not persuade the reader to follow a specific course of action. The responses should be factual, objective, and only consist of scientifically balanced information identified in the literature search or other appropriate internal sources (data on file) [ 32 ].

When an MI scientist uses their therapeutic area expertise and knowledge on drug therapy to respond by phone or in-person (e.g., at a Medical Congress), it is essential to demonstrate professional courtesy, empathy, and employ active listening techniques. As the MI scientist prepares a narrowly tailored-specific response, they also must be prepared for additional follow-up queries.

The MI scientist is a customer-facing role where they are acting on behalf of the company. The MI scientist has a professional and ethical responsibility to provide evidence-based, scientifically balanced, and non-misleading answers to MI requests [ 32 , 33 ].

Medical information professionals in the pharmaceutical industry have a unique responsibility to provide accurate, unbiased, evidence-based, and scientifically balanced information about their company’s product(s) to patients and HCPs. The phactMI code of practice (COP) describes MI science skills as the technical acumen that medical information professionals are equipped with to carry out their responsibilities effectively [ 2 ]. Aside from their technical skills, MI professionals often hold advanced healthcare degrees and have relevant clinical expertise [ 34 ]. Given the scientific and methodical nature of their work, MI scientist may be a better descriptor for the role.

This skillset that MI scientists must utilize can be described by the acronym DRESS (defining the question, researching the topic, evaluating the evidence, synthesizing a response, and sharing the answer). The rigorous process by which MI scientists respond to unsolicited medical information requests parallels the scientific method. Every scientific experiment starts with a question or hypothesis, for example, is drug A safe and effective in treating condition B? To ensure full comprehension of the topic, the scientist must have background knowledge through research of the subject. The necessary background can help define the confounding variables. Next, the researcher must design an experiment and collect the required data. They then carefully evaluate the data and determine the answer to the research question based on the evidence. After drawing a conclusion, the researcher may choose to communicate the findings in a scientific report.

MI scientists work in a similar manner. When Dr. Doe, a hospital pharmacist, sends a request to the medical information department responsible for product B, five key steps are undertaken to send him an accurate and scientifically balanced response. These steps are similar to the scientific method:

  • The MI scientist begins the process by defining the question . Often this step requires background knowledge of the topic.
  • Once the question is well defined, the MI scientist researches the topic in the biomedical literature for relevant data. MI scientists are well versed in utilizing different databases to build a robust search strategy for a given topic.
  • After gathering the available data, the next step is to evaluate the evidence . MI scientists use their expertise to evaluate and critically appraise clinical data, keeping in mind that not all levels of evidence are the same. Utilizing their knowledge of the literature and professional judgment, MI scientists respond to inquiries with the highest level of evidence available.
  • Once the evidence has been evaluated, the next step is synthesizing a response . This requires the skill and understanding of who is asking the question and the channel through which the response will be distributed.
  • The final step then is sharing the answer with the requestor in a clear, accurate, balanced, and succinct manner.

Using the DRESS approach can organize the process by which MI scientists make available the right information to clinicians in a format for decision-making.

Professionals in the medical information departments of pharmaceutical companies are highly trained individuals uniquely positioned to provide accurate and scientifically balanced information about their drug(s). The methodological approach followed by MI scientists to respond to MI requests parallels the scientific method. Following the DRESS approach, the MI scientist can effectively and efficiently respond to the information needs of customers, thereby supporting optimal patient care and outcomes.

The queen of the line is the Balance 526 that was introduced a few years ago and has proven to be a very successful player in the 50-foot plus segment of the cat market. Built in South Africa by noted composite experts, the 526 offers a well-conceived combination of great sailing performance and luxury living in a boat that can be handled by an experienced couple.

The boat has plenty of innovations, among them a helm that can be tilted up so you can steer from the raised helm station or tilted down so you can steer from the protection of the cockpit. Very cool. The design does not take any particular element to the “extreme.” Instead, Berman and his crew have sought to provide a boat that is truly well balanced in all aspects. And, they’ve done it.

Manufactured in 2014, this luxury catamaran is ideal for those who enjoy watersports and healthy activities. It was designed by world-renowned designer Van Peteghem-Lauriot Prvost (VPLP) and built to top-notch quality with a focus on stability, performance, luxury, and spaciousness. The maximum capacity of this boat is 12 guests in 4-double and 2-twin cabins. The Sunreef 620 is the very first to feature a newly developed rig. From the large cabins, saloon, galley, and sail performance, the 620 is one boat you’d get a memorable experience on. Aimed at Blue-water sailors looking to sail fast and far, Switch 51 has proved itself and regarded in this day as a remarkable ocean cruising catamaran. This classic high-performance voyage was designed by VPLP and is said to combine a level of comfort, performance, and safety. It was built by Sud Composites in France with high tech materials. Sud initially built about 18 of these classic catamarans from 2001 to 2007.

Switch 51 hulls are built with vinyl ester sandwiched with a Klegecelle PVC foam core, thereby creating a hull light enough to be real quick and heavy enough to handle the ocean waves and swells. The saloon and galley are huge, with the cockpit spacious. The layout and accommodation are perfect for a small family with classic amenities aimed at making your stay on board a memorable one. This also a pretty affordable option in this category with prices on the used market as low as 450k.

Strange Glow Over Moscow Skies Triggers Panic as Explosions Reported

B right flashes lit up the night sky in southern Moscow in the early hours of Thursday morning, new footage appears to show, following reports of an explosion at an electrical substation on the outskirts of the city.

Video snippets circulating on Russian-language Telegram channels show a series of flashes on the horizon of a cloudy night sky, momentarily turning the sky a number of different colors. In a clip shared by Russian outlet MSK1.ru, smoke can be seen rising from a building during the flashes lighting up the scene.

Newsweek was unable to independently verify the details of the video clips, including when and where it was filmed. The Russian Ministry of Emergency situations has been contacted via email.

Several Russian Telegram accounts said early on Thursday that residents of southern Moscow reported an explosion and a fire breaking out at an electrical substation in the Leninsky district, southeast of central Moscow.

Local authorities in the Leninsky district told Russian outlet RBC that the explosion had happened in the village of Molokovo. "All vital facilities are operating as normal," Leninsky district officials told the outlet.

The incident at the substation in Molokovo took place just before 2 a.m. local time, MSK1.ru reported.

Messages published by the ASTRA Telegram account, run by independent Russian journalists, appear to show residents close to the substation panicking as they question the bright flashes in the sky. One local resident describes seeing the bright light before losing access to electricity, with another calling the incident a "nightmare."

More than 10 villages and towns in the southeast of Moscow lost access to electricity, the ASTRA Telegram account also reported. The town of Lytkarino to the southeast of Moscow, lost electricity, wrote the eastern European-based independent outlet, Meduza.

Outages were reported in the southern Domodedovo area of the city, according to another Russian outlet, as well as power failures in western Moscow. Electricity was then restored to the areas, the Strana.ua outlet reported.

The cause of the reported explosion is not known. A Telegram account aggregating news for the Lytkarino area described the incident as "an ordinary accident at a substation."

The MSK1.ru outlet quoted a local resident who speculated that a drone may have been responsible for the explosion, but no other Russian source reported this as a possible cause.

Ukraine has repeatedly targeted Moscow with long-range aerial drones in recent months, including a dramatic wave of strikes in late May.

On Sunday, Moscow Mayor Sergei Sobyanin said the region's air defense systems had intercepted an aerial drone over the city of Elektrostal, to the east of Moscow. No damage or casualties were reported, he said.

The previous day, Russian air defenses detected and shot down another drone flying over the Bogorodsky district, northeast of central Moscow, Sobyanin said.

There is currently no evidence that an aerial drone was responsible for the reported overnight explosion at the electrical substation in southern Moscow.
